This form is for bands interested in being considered as support for a show at The State Theatre.
Please fill out ALL the fields completely. Include your band's history in the area and any attendance information related to it (remember, we can check on these numbers!)
Please do NOT send press kits in the mail unless specifically asked.
It is not necessary to send multiple forms unless your information changes. Once you send it - we've got it, and will keep it on file.
If an opportunity comes up to have your band at The State, we will contact you at the contact email on this form. If a specific show on our calendar stands out to you as something you would like to be considered for, please do not hesitate to mention it in the comments section.
